Somnoware, a leading provider of digital health technology, announces the launch of a new Care Management Module that empowers providers to transition to outcomes-based care. By uniting data from Somnoware’s diagnostic platform with nightly PAP therapy data in an intuitive interface, this module allows medical directors and their care teams to holistically evaluate their patient population so that they can effectively allocate and target care.

Providers using this module have access not only to centralized compliance data for patients on PAP therapy, but also a toolkit for assessing risk and the outcomes of interventions over time. Therapy data for the patient population can be viewed and grouped according to diagnostic AHI and comorbidities, as well as by trajectory towards meeting compliance goals. Risk stratification will be further enhanced by machine learning models that the company has developed. Physicians and medical directors using this module will be presented with a dashboard divided into four primary components–each with the ability to drill down into a specific population and generate a corresponding list, which can then be filtered and exported:

  1. Treatment vs. Episode of Care: Physicians can take full advantage of Somnoware’s end-to-end integrations to readily identify gaps of care in onboarding to PAP therapy, compared to a baseline event—including diagnostic studies, titration studies, and DME orders. These events are taken in from Somnoware’s diagnostic platform and HL7 feeds. Patients who are not set up on therapy can be viewed monthly and then triaged to determine the reason for any gap in care and followed up with accordingly.
  2. At-Risk Profile: Providers can prioritize and direct care where it is most needed by viewing a rolling 90-day view into patients who have been recently set up on therapy. These patients are separated into those who are At-Risk of failing their compliance goal and those who are already On Track toward completing it, giving an instant indicator of onboarding efficacy. At-Risk patients are further stratified by diagnostic AHI to rank sleep apnea severity.
  3. Comorbidities: The link between sleep and associated health conditions is being increasingly recognized, yet few sleep tools give insight into patients’ holistic medical status. Through this module, physicians can visualize which of their patients have key comorbidities associated with sleep apnea, and how many have multiple conditions so that they can contextually allocate care according to the specific needs of their patients. A predictive risk score is being developed that incorporates cardiovascular disease, hypertension, congestive heart failure, chronic obstructive pulmonary disease, coronary artery disease, and diabetes mellitus—among other comorbidities.
  4. Compliance: Lastly, physicians can track the efficacy of their interventions by monitoring the compliance of their patient population. By linking historical with nightly data, physicians can view trends and act toward practice improvement.

Each component of the module’s dashboard is fully interactive and populates a corresponding list of patients, sorted by the severity of their diagnostic AHI. This list can be expanded and filtered to further segment the patient population, prior to intervention. Sleep physicians and care teams can seamlessly navigate between the patient list and a detailed individual patient view that gives an instant snapshot of diagnostic, compliance, and device data, along with the ability to view PAP usage, leak, and AHI trends. Patient contact information is readily available for any necessary follow-up.
